Applying for Polish Visa


Students that will be spending a semester in Poland must apply for a student visa.  It is recommended that students apply for a long-stay multiple entry visa.  Visa applications and supporting documents may be mailed to the Polish Consulate in Chicago.  The visa processing fee is free for citizens of theUnited States. 

Required Documents
  • Passport.  Must be signed and valid for at least 3 months from the date of intended departure from Poland, with at least one blank visa page.
  • Visa Application Form filled out and signed by the applicant.  All entries of the form should be properly filled out (no blanks).
  • Two passport size photographs (available at Padnos International Center for $2)
  • Notarized copy of a US Permanent Resident Card (green card) or other document confirming status in the US (drivers license is acceptable)
  • Bank statement – 3 most recent statements
  • Confirmation letter from health insurance company stating coverage with a minimum of 30,000 EURO (equivalent in USD) for trips outside the US
  • Reference letter from employer
  • Letter of acceptance from a Polish university/language school/college (a copy of this letter is sufficient, you should keep the original to present to customs upon arrival if asked to do so)

The documents listed above should be submitted by US Postal Service with your passport.  It is best to insure your package in the event that your mail is lost or stolen.  You must also include a self addressed stamped envelope (preferably insured) in order for your passport and visa to be returned to you. Applications, required documents, and passports may be mailed to the Polish Consulate in Chicago. Please allow a minimum processing time of 15 business days.
